Abstract

Background & Objective: The standard rate of cesarean delivery, from the perspective of the World Health Organization is 15 percent, which in most places is very different from the amount recommended by the World Health Organization. One of the most effective solutions to reduce the incidence of cesarean section is vaginal delivery after cesarean and reduce repeat cesarean. Therefore, the present study is done to prevalence and some maternal and neonatal outcomes of vaginal delivery after cesarean and Repeat Cesarean in a hospital repeated Bentolhoda Bojnoord.

Materials& methods: Descriptive and analytical study on 180 mothers eligible performed, in 1392 and in North Khorasan University of Medical Sciences, Bentolhoda Hospital. Demographic questionnaire, details of delivery, maternal& neonatal outcomes are Study Tools. Spss software, version 18 descriptive statistics for tables, mean, Charts, variance, standard deviation, was used.

Results: % 47/8  of mothers were able to do a vaginal birth after cesarean. Bleeding and fever from mother outcom and neonatal outcomes with Fisher test was not significant, but in vaginal delivery after cesarean group ,uterin rupture in first stage and hysterectomy constipation was less than repeat cesarean that in constipation the difference between groups was significant.

Conclusion: With careful selection to reduce unnecessary cesarean, can used vaginal delivery after cesarean.

Abstract

Background & Objectives: According to the report by Iran Drug Control Headquarters, 657 thousand drug-dependent patients are under maintained treatment in Iran. The aim of present study was investigation of treatment-retention in patients who were undergone the maintained therapy using Methadone, Buprenorphine, and opium tincture in Shirvan city. It should be noted that the treatment with opium tincture is done only in Iran. Therefore, the success of this method is a controversial issue.

Material & Methods: This is an analytical-descriptive study. The population of this study were included all active patients and also patients who have abandoned treatment in 6 public and private addiction treatment centers of Shirvan city in 1394. Data were analyzed by SPSS software, and chi-square test was used to compare the qualitative variables.

Results: Retention rate of Methadone, Buprenorphine, and opium treatments were 84.5%, 71.7%, and 90.1%, respectively. A significant difference was observed among these treatments, so the opium maintained treatment has the highest retention rate.

Conclusion: The results revealed that the retention rate in treatment with opium tincture is higher than that in the other two methods. If we focus on the goals of harm reduction programs, results show that the opium tinctures treatment can reduce significantly the amount of social damage.

Abstract

Background & Objectives: Posterior cruciate ligament is very important in stabilizing knee joint and its rupture and avulsion may result in instability and joint degeneration. The aim of this study was to evaluate the operation outcome of PCL ligament binding site avulsion.

Material & Methods: In present study about 30 patients with avulsion fracture on Tibial PCL binding site were undergone the open surgery and fixation with screw. The patients were followed up for at least 6 months after the operation and were evaluated by the Lysholm Knee Score checklist.

Results: The mean age of patients was 28 years. The common injury mechanism for these patients was motor accident with machine (MCA). On the Lyshom Knee Score, twenty one persons (70%) scored as good (60-90) and 9 (30%) cases were scored as fair (30-59), with a mean score of 86.

Conclusion: Clinical outcome achieved from physical examination of the knee and the checklist score, suggests that the posterior cruciate ligament avulsion fracture injury is not solely a bone fracture and should be viewed as bone-ligament injury. It seems that operation approach is an appropriate method to improve knee function and reduce the complications.

Abstract

Introduction: The present study investigated the relationship between hematological parameters and clinical outcomes in COVID-19 patients.
Method: This cross-sectional study was carried out on 293 adult patients with COVID-19 hospitalized in Imam Hassan Hospital (Bojnurd, Iran), and less than three days had passed since the onset of COVID-19 symptoms. Patients information, including demographic information, clinical characteristics, and clinical outcomes, were obtained from electronic medical records. The results related to the complete blood count test for each patient were also recorded.
Results: The median age of patients was 53 years, and 147(50.17%) were female. Thirty-seven patients were admitted to the ICU, and 42 patients died. The mean of hematological parameters, except neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and erythrocyte sedimentation rate (ESR), in COVID-19 patients were within the normal range. We found that values of white blood count (WBC), NLR, and PLR in patients who died or were admitted to the ICU were significantly higher than in patients who were not admitted to ICU (P˂0.05). Moreover, our results indicated hemoglobin, hematocrit, and ESR levels were lower in patients who died than in patients who were not admitted to ICU (P˂0.05). No significant relationship was found between hematological parameters and the length of hospital stay in patients with COVID-19 (P˃0.05).
Conclusion: The obtained results suggest that increased values of some hematological parameters, including WBC, NLR, and PLR, at the time of admission might predict poor clinical outcomes in COVID-19 patients
