fa بررسی میزان مرگ و میر با استفاده از سیستم نمره بندی استاندارد شدت بیماری آپاچی دو در بیماران بستری در بخش مراقبت ویژه بیمارستان شهید دکتر چمران شهر فردوس درسال 1395 Mortality Rate of Patients Admitted to the Intensive Care Unit of Shahid Chamran Hospital of Ferdows City with Acute Physiology and Chronic Health Evaluation (APACHE) II علوم پایه Basic Sciences مقاله پژوهشی Orginal Research <div style="text-align: justify;">مقدمه: اولویت بندی پذیرش بیماران بدحال در بخش مراقبت ویژه در کاهش مرگ و میر و مدت زمان بستری نقش مهمی دارد. هدف این مطالعه ارزیابی میزان مرگ و میر بیماران بستری در بخش مراقبت ویژه بیمارستان دکتر چمران فردوس با استفاده از معیار آپاچی دو است. روش کار: در این مطالعه توصیفی مقطعی 80 بیمار بستری در بخش مراقبت ویژه بیمارستان دکتر چمران فردوس به روش نمونه گیری در دسترس از بهمن 1394 تا بهمن 1395 وارد مطالعه شدند. ابزار، پرسشنامه دموگرافیک و سیستم نمره بندی استاندارد شدت بیماری (آپاچی) دو بود. زنده ماندن یا فوت شدن و طول مدت بستری ثبت شد. داده‌ها با استفاده از نرم افزار SPSS نسخه 21 با استفاده از آزمون‌های آماری چون تی مستقل مورد تجزیه و تحلیل قرار گرفت. یافته‌ها: میزان مرگ و میر درکلیه بیماران 40 درصد و مرگ و میر پیش بینی شده توسط مقیاس آپاچی دو 15 درصد به دست آمد. میانگین نمره آپاچی دو بیماران زنده مانده 8/4 ± 9/14 و بیماران فوت شده 6/4 ± 17 حاصل شد که این تفاوت از نظر آماری معنی دار بود (05/0 P <). بین میانگین نمره آپاچی دو و میانگین مرگ و میر در بیماران رابطه آماری معنی داری مشاهده شد (05/0 P < 220/0 = r). نتیجه گیری: درصد مرگ و میر واقعی بیماران با درصد تخمینی توسط آپاچی دو فاصله دارد. نتیجه گیری در این خصوص، نیازمند بررسی بیشتر است.  </div> <div style="text-align: justify;">Introduction: Prioritization in the admission of critical patients at the intensive care unit plays an important role in reducing mortality and hospital stay. The aim of this study was to determine the mortality rate of patients admitted to the Intensive Care Unit (ICU(of Shahid Chamran Hospital of Ferdows City with Acute Physiology and Chronic Health Evaluation (APACHE) II. Methods: In this cross-sectional study, 80 patients admitted to the Intensive care unit of Shahid Chamran Hospital of Ferdows were selected through the convenience sampling method, according to the inclusion criteria from January 2016 to January 2017. The tool of the study included demographic questionnaire and APACHE II. The survival or death of patients and the length of ICU stay were recorded. Data were analyzed with the Chi-square and independent t-test using SPSS version 21. Results: The mortality rate among all patients was 40% and the predicted mortality rate with the APACHE score was 25%. The mean APACHE II score was 14.9 ± 8.4 and 17 ± 4.6 in the surviving patients and the dead patients, respectively. The difference between them was significant (P value < 0.05). There was a significant correlation between APACHE II score and the mortality rate of patients (P value < 0.05, r = 0.220). Conclusions: The actual mortality percentage was far from the estimated percentage calculated by APACHE II.
