fa تأثیر متغیرهای قبل از آزمایش بر تشخیص اختلالات متابولیسم مادرزادی؛ یک مطالعه مروری بالینی Clinical مقالات مروری Review Article <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:nasimYW;"><strong>مقدمه</strong>: بررسی پروفایل متابولیتها در نمونههای بیولوژیکی منعکس کننده وضعیت فیزیولوژیکی و شناسایی اختلالات مرتبط با مسیرهای متابولیکی میباشد. متغیرهای قبل انجام آزمایش حدود 12 تا 19 درصد از خطاهای آزمایشگاهی را شامل میشود و از زمان درخواست آزمایش توسط پزشک تا آماده شدن نمونه برای آنالیز را در بر میگیرد. هدف این مطالعه بررسی اثر متغیرهای قبل از انجام آزمایش در تکنیکهای تشخیصی اختلالات متابولیسمی مادرزادی میباشد.<br> <strong>روش کار:</strong> در این مطالعه 19 مقاله با واژگان کلیدی خطاهای قبل از انجام آزمایش، اختلالات متابولیک مادرزادی، نمونه، طیف سنجی جرمی، کروماتوگرافی گازی در پایگاههایی نظیر PubMed ، scholar و Scopus از سال 3561 تا کنون بررسی گردید.<br> <strong>یافته ها:</strong> تعداد زیادی از خطاهای قبل از انجام آزمایش وجود دارند که میتواند به طور جدی اعتبار نتایج را در اختلالات متابولیک مادرزادی تحت تأثیر قرار دهد. این خطاها در درجه اول شامل جمعآوری نمونه، بررسی نمونه و متغیرهای فیزیولوژیک شامل شیوه زندگی، رژیم غذایی، استرس، سن، جنس، موقعیت اجتماعی و متغیرهای خارجی شامل داروها و آنتیبادیهای مورد استفاده میباشد.<br> <strong>نتیجه گیری: </strong>با توجه به اینکه متغیرهای قبل از انجام آزمایش میتواند اثرات غیرقابل پیش بینی و نامطلوب روی سلامت بیمارداشته باشد، کاهش در این خطاها نقش مهمی در برنامه ارزیابی و بهبود کیفیت ایفا خواهد نمود. بدین منظور نمونه با کیفیت مناسب که نتیجه آموزش مناسب و آگاهی از فاکتورهای مؤثر بر نتایج آزمایشگاهی میباشد، جهت به حداقل رساندن خطاها و بهینه سازی استفاده از منابع، کیفیت نتایج و در نهایت بهبود کل فرآیند مدیریت بیمار و اطمینان از تشخیص دقیق ضروری میباشد.</span></span></div> <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Arial;"><strong>Introduction:</strong> Quantitative metabolite profiling in biological samples has the potential to reflect physiological status and to identify disease associated disorders in metabolic pathways. This approach is hindered by a wide range of pre-analytical variables. Pre-analytical variables account for 32-75% of laboratory errors, which includes the time from when the test is ordered by the physician until the sample is ready for analysis. The aims of this study was to evaluate the pre-analytical errors in diagnostic methods of inherited metabolic disorders.<br> <strong>Methods:</strong> In this study, 35 articles with the key words Pre-analytical Errors, Metabolic disorders, Samples, Mass Spectrometry, and Gas Chromatography were searched in PubMed, Google Scholar, and Scopus databases from the year 1987 to 2018.<br> <strong>Results:</strong> There are some less controllable sources of pre-analytical errors that can strongly influence the reliability of test results in inborn errors of metabolism. These primarily include specimen collection, handling samples and physiological variables such as the effect of lifestyle, diet, stress, age, gender, positional effects, and endogenous variables such as drugs and circulating antibodies.<br> <strong>Conclusions:</strong> As pre-analytical sources of variation can produce unpredictable and unfavorable impacts on the wellbeing of patients, a reduction in laboratory testing errors play a significant role in assessing and quality improvements.
