fa بررسی ارتباط بین شاخص‌های خون‌شناسی و نتایج بالینی در بیماران مبتلا به کووید 19 بستری در بیمارستان امام حسن بجنورد در اردیبهشت تا مرداد 1400 Evaluation of the Relationship between Hematological Parameters and Clinical Outcomes in COVID-19 Patients Hospitalized in Imam Hassan Hospital of Bojnurd, Iran, April-August 2021 علوم پایه Basic Sciences مقاله پژوهشی Orginal Research مقدمه: در مطالعه حاضر رابطه بین شاخص‌های خونی و پیامدهای بالینی در بیماران کووید 19 بررسی شد. روش کار: این مطالعه مقطعی روی 293 بیمار بزرگ‌سال مبتلا به کووید 19 بررسی شدند که در بیمارستان امام حسن بجنورد، ایران بستری شده بودند و کمتر از سه روز از شروع علائم کووید 19 آن‌ها گذشته بود. از پرونده الکترونیکی، اطلاعات بیماران شامل اطلاعات دموگرافیک، مشخصات بالینی و نتایج بالینی استخراج شد. نتایج مربوط به آزمایش شمارش کامل خون برای هر نمونه نیز ثبت شد. یافته‌ها: میانه سن بیماران 53 سال بود. 147 نفر 17/50 درصد از بیماران زن بودند. 37 بیمار در آی‌سی‌یو بستری بودند و 42 نفر نیز فوت کردند. میانگین شاخص‌های خون‌شناسی به‌جز نسبت نوتروفیل به لنفوسیت NLR، نسبت پلاکت به لنفوسیت PLR و میزان رسوب اریتروسیت ESR در بیماران کووید 19 در محدوده نرمال قرار داشت. مقادیر گلبول سفید WBC، NLR و PLR در بیمارانی که فوت کرده یا در آی‌سی‌یو بستری بودند، به‌طور معناداری بیشتر از بیمارانی بود که در آی‌سی‌یو بستری نشدند 05/0˂P. همچنین نتایج نشان داد مقادیر هموگلوبین، هماتوکریت و ESR در بیماران فوت‌شده نسبت به بیمارانی که در آی‌سی‌یو بستری نشده بودند، کمتر است 05/0˂P. بین شاخص‌های خون‌شناسی و مدت‌زمان بستری در بیمارستان ارتباط معناداری مشاهده نشد 05/0 ˃P. نتیجه گیری: به‌طورکلی نتایج نشان داد مقادیر افزایش‌یافته برخی از شاخص‌های خون‌شناسی همچون WBC، NLR و PLR در زمان بستری ممکن است پیامد بالینی ضعیف را در بیماران مبتلا به کووید 19 پیش‌بینی کند. <div style="text-align: justify;">Introduction: The present study investigated the relationship between hematological parameters and clinical outcomes in COVID-19 patients. Method: This cross-sectional study was carried out on 293 adult patients with COVID-19 hospitalized in Imam Hassan Hospital (Bojnurd, Iran), and less than three days had passed since the onset of COVID-19 symptoms. Patients information, including demographic information, clinical characteristics, and clinical outcomes, were obtained from electronic medical records. The results related to the complete blood count test for each patient were also recorded. Results: The median age of patients was 53 years, and 147(50.17%) were female. Thirty-seven patients were admitted to the ICU, and 42 patients died. The mean of hematological parameters, except neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), and erythrocyte sedimentation rate (ESR), in COVID-19 patients were within the normal range. We found that values of white blood count (WBC), NLR, and PLR in patients who died or were admitted to the ICU were significantly higher than in patients who were not admitted to ICU (P˂0.05). Moreover, our results indicated hemoglobin, hematocrit, and ESR levels were lower in patients who died than in patients who were not admitted to ICU (P˂0.05). No significant relationship was found between hematological parameters and the length of hospital stay in patients with COVID-19 (P˃0.05).
