Abstract Background & Objectives: Medical records is one of the most important sources access to information about diseases , their treatment , management and performance of health care staffs. The evaluation of medical records services of hospitals could be a powerful means to promote the quality and quantity of health systems. The aim of this study was to determine the status of the medical records department of the hospitals in North Khorasan province, in order to identify their strengths and weaknesses and report to the relevant authorities. Materials and methods: This is a descriptive cross-sectional study. The performance of the medical records of 6 hospitals related to North khorasan University of Medical Sciences was evaluated during first three month in2013. Data collection was performed by direct attendance of researcher to the departments and filling out the checklist. Data were analyzed by SPSS software. Result: A total of 45 active staff in the medical records department, 71% was women. 56% have college education and 44% did not have academic education. Performance of the medical records were obtained %89.47. Filing axis showed the highest score (97/5%) and item of test competency and empowerment performance score showed the lowest score (52/6%). Discussion: The medical record showed favorable performance status. Axis of test competency, empowerment, data collection and analysis showed the lowest scores. Careful planning for training of personnel and empowering in order to reach a agreeable performance is recommended .
