Introduction: Osteocalcin (OCN) plays a role in regulating glucose metabolism and insulin sensitivity; however, the impact of aerobic exercise on OCN levels in adult women remains inconsistent across studies. This systematic review and meta-analysis aimed to evaluate the effects of aerobic exercise on osteocalcin levels and insulin resistance in adult women.
Methods: A systematic search was conducted across major databases, including PubMed, Scopus, Web of Science, Google Scholar, NoorMags, Magiran, and SID, for studies published up to January 18, 2025. Studies comparing aerobic exercise with a control group on OCN and insulin resistance were included. Quantitative data, such as means, standard deviations, and sample sizes, were extracted. A meta-analysis was performed using a random-effects model, and the standardized mean difference (SMD) with 95% confidence intervals (CI) was calculated via Comprehensive Meta-Analysis (CMA2) software to determine the effect size.
Results: Meta-analysis of 8 studies involving 275 adult women demonstrated that aerobic exercise significantly increased serum OCN levels [SMD=1.841 (95% CI: 0.524 to 3.159); P=0.006] and significantly reduced insulin resistance [SMD=-1.011 (95% CI: -1.852 to -0.171); P=0.018] compared to the control group.
Conclusion: Aerobic exercise is an effective approach for enhancing key metabolic health markers in adult women. These findings highlight the necessity of integrating aerobic activities into health-oriented and physical activity programs for this population
